“No one is permitted to have access/review a patient’s chart (paper or electronic) if they are not involved in their circle of care, without having consent from the person in question or their
substitute decision maker. However, there may be specific (eg. emergency situations) where a patient/client consent may or may not be required depending on the circumstances.”
