Work Situation Report

General Information

This Work Situation Report was developed jointly by Representatives of the PEI Nurses’ Union and The Employer. The Work Situation Report Form represents a Provincial standard, and will be utilized by all worksites/work units where PEI Nurses’ Union members are employed, (as indicated in Article 16.7 of the present Collective Agreement) and, therefore, it is not to be changed by any one individual, or organization.

Guidelines for Use

The Work Situation Report is a form to be used to document concerns about patient / client care in the workplace. It does not replace the worksite/work unit’s Incident Report Form.

The purpose of the Work Situation Report is to:

  1. Provide an avenue whereby the staff members can document and bring concerns to the Employer’s attention for appropriate action; and
  2. Provide a formal record of concerns about specific problems for future verification, if necessary.

Things to Remember

  1. Complete the form legibly.
  2. The form is to be completed by the person(s) involved. DO NOT complete a report for someone else.
  3. DO NOT identify patients/clients or professional involved in the incident being described.
  4. The report form should be completed and submitted within 24 hours of the situation.
  5. As much as possible, report only the facts about which you have first-hand knowledge. If more space is required, please add additional page(s).
  6. The staff member completing the form must ensure that the report reached Nursing Administration, who in turn, will forward a copy to the Local President or designate.

Education Fund

The purpose of the education fund is to support continuing education by providing funding to enable participation in workshops, seminars and short courses including CNA certification and recertification programs (not study groups).

Reimbursements may be made for university, college and post-graduate courses/ programs related to the profession.

The Education Fund is not meant to replace employer funding for mandatory courses, required orientation or training needed to expand a nurses level of competency to work within the worksite/ service delivery.

Memberships to various associations are not eligible for reimbursement, unless at the time of the request, the membership results in a reduced course cost (greater than or equal to the cost of the membership fee).

If the employee is signing a return to service agreement when taking a course, this is not eligible for reimbursement through Education Fund.

All PEINU members are invited and are encouraged to attend the Fall Education Day on October 30th at Murchison Centre, Charlottetown.

Online Registration for the Fall Education Day is open until October 23, 2019.

Note: There will be a $20 Registration Fee required PRIOR to the October 30th event.

PEINU is offering two options for a member to use to make the Registration Fee payment:

  1. Interac e-Transfer (instructions at bottom of page); or
  2. Payment can be dropped off at the PEINU Provincial office, 10 Paramount Drive, Charlottetown, PEI. Office hours are Monday through to Friday, 8am-4pm (closed from 12pm-1pm for lunch) or payment in an envelope with your name and address can be dropped into the mail slot at the PEINU office at any time and a receipt will be mailed to the registrant.

Members who register after the 125 seat limit is reached will be wait listed for potential cancellations and will not be required to pay the $20 fee until the member is offered a seat. Members will be offered seats in the order that the wait list registration was received.
